# How to add our gift form to checkout page to allow customer give or buy for someone else

This page describes the old product-template route for the gift form and points at the current guide

**This page is out of date.** It described adding the gift form from the Online Store theme editor, back when the form was a product-template block. That is no longer how the gift form works, so the old steps have been removed instead of left here to mislead you.

The gift form is a checkout UI extension now. You add it in the **checkout editor** (Shopify admin -> **Settings** -> **Checkout** -> **Customize**), not in the theme editor, and checkout extensibility needs a Shopify plan that supports it. In the app you also have to switch the feature on first, in **Settings** -> **Checkout page**, before the form appears to customers at all.
